Проведен анализ ассоциаций полиморфного маркера A49G гена цитотоксического Т-лимфоцит-связанного иммуноглобулина 4 (CTLA4) с БГ и АИТ с исходом в гипотиреоз. У больных БГ гомозиготный генотип GG и аллель G встречается достоверно чаще, чем в группе здоровых (р = 0,032, р = 0,016 соответственно). Аллель G и генотип GG ассоциированы с повышенным риском наличия БГ в новосибирской популяции — ОIII = 1,6, 95% ДИ, 1,1—2,2; ОIII = 2,4, 95% ДИ, 1,2—5,0 соответственно. Выявлены гендерные различия в частоте генотипов и аллелей A49G гена CTLA4 у больных БГ. В группе больных мужчин гомозиготный генотип GGвстречается достоверно чаще, чем в группе здоровых: 34 против 13%; χ                  2 = 7,9; р = 0,005), носительство аллеля G также достоверно чаще, чем в группе здоровых: 61 против 41%; χ                  2 = 6,31; р = 0,012. Обнаруженные особенности в ассоциации аллеля G и генотипа GG с повышенным риском БГ отмечены лишь у мужчин (ОШ = 2,2 и 9,1 соответственно). У женщин подобных особенностей не отмечено. У больных с аутоиммунным тиреоидитом с исходом в гипотиреоз (как у мужчин, так и у женщин) по сравнению с контрольной группой не выявлено достоверной разницы в частотах генотипов и аллелей полиморфизма A49G изучаемого гена.</p></abstract><trans-abstract xml:lang="en"><p>Autoimmune thyroid disease (AITD) is caused by an immune response to self-thyroid antigen. The cytotoxic T-lymphocyte antigen-4 (CTLA4) gene, encoding a negative regulator of the T-lymphocyte immune response, had been reported to be associated and/or linked to AITD. The aim of the study was to investigate the association between the exon 1 CTLA-4 gene polymorphism A(49)G and susceptibility to graves’ disease (GD) and Hashimotos thyroiditis (HT). Materials and methods. We analyzed the A(49)G exon 1 CTLA-4 gene polymorphism in 105 unrelated Novosibirsk patients with GD, in 101 patients with HT and 150 matched healthy subjects. Results. The distribution of genotype frequencies differed significantly between patients with GD and controls (p = 0.039). The allele G and the genotype GG were associated with the increased risk for GD (odds ratio OR = 1,55, 95% CI, 1,09—2,21, OR = 2,43, 95% CI, 1,17—5,01 respectively). In contrast, no differences in genotype frequencies were observed between HT patients and controls for the A(49)G exon 1 CTLA-4 gene polymorphism.
