Выявление значимости наличия мутации V600E гена BRAF в дооперационной диагностике рака щитовидной железы. Материал и методы. В исследование включены 46 пациентов с узловыми образованиями щитовидной железы, которым была проведена тонкоигольная аспирационная биопсия (ТАБ). ДНК выделялась сорбентным методом из материала ТАБ. Выявление мутации гена BRAF проводилось с помощью ПЦР с праймерами, характерными для дикого и мутантного типа гена. Результаты. По результатам цитологического исследования все пациенты были разделены на 3 группы: 1-я – больные с узловым коллоидным зобом (УКЗ) (9), 2-я – больные папиллярным раком (ПР) (19) и 3-я – пациенты с неопределенными результатами ТАБ (18). В группе пациентов с ПР все диагнозы были подтверждены гистологически, и мутация гена BRAF была выявлена у 15 (79%) пациентов. В группе с неопределенным цитологическим диагнозом только у одного пациента определена мутация гена BRAF и при гистологическом исследовании был диагностирован фолликулярный рак. У 16 пациентов с гистологическим диагнозом “фолликулярная аденома”, у 9 пациентов с УКЗ и у 1 пациента с фолликулярным раком мутации BRAFV600E выявлено не было. Таким образом, мы получили следующие критерии достоверности метода: чувствительность – 76%, специфичность – 100% и диагностическая точность – 89%. Выводы. Выявление мутации BRAFV600E может быть использовано в качестве дополнительного метода дооперационной диагностики РЩЖ.</p></abstract><trans-abstract xml:lang="en"><p>Purpose: To assess the utility of BRAFV600E mutation detection in preoperative thyroid cancer diagnostic. Material and methods: We studied 46 aspirates taken by FNA from patients with thyroid gland nodes. Thyroid tissue aspirates DNA was extracted by sorbent method. BRAF gene mutation was analyzed with primers specific for wild and mutant gen type by RT-PCR. Results: All patients were divided into three groups by cytological conclusions: colloid nodules (9), PTC (19), and suspicious for malignancy (18). In the group of patients with PTC all diagnoses were confirmed by histology, and BRAF gene mutation was detected in 15 (79%) FNAB specimens. In the group with suspicious cytological diagnosis only one patient had follicular cancer by histology and positive BRAF mutation. There were detected no BRAF mutation in 16 patients with histologically proven follicular adenoma, in 9 patients with colloid nodular goiter and in one patient with follicular cancer. Thereby, we received the following criteria valuers of method’s reliability: sensitivity – 76%, specificity – 100%, diagnostic accuracy – 89%.
