Своевременная диагностика такого серьезного осложнения, как парез гортани, остается актуальной по сей день. Основным методом диагностики пареза гортани до сих пор является эндоскопическая ларингоскопия, однако она имеет ряд недостатков: процедура является инвазивной, причиняя дискомфорт и неудобства пациенту, для проведения ларингоскопии требуется местная анестезия, что может сопровождаться аллергической реакцией, из-за анатомических особенностей у некоторых пациентов ларингоскопия невыполнима. Все вышеперечисленное заставляет искать более простой и в то же время эффективный метод диагностики нарушений движения голосовых складок гортани.</p><p>В данной работе представлен опыт выполнения 1252 чрескожных ультрасонографий голосовых складок. Показаны диагностическая эффективность и удобство выполнения данной методики в условиях отделения эндокринной хирургии.</p><sec><title>Цель</title><p>Цель.</p><p>Оценить диагностические возможности чрескожной ультрасонографии для определения подвижности голосовых складок у пациентов после оперативных вмешательств на щитовидной и околощитовидных железах в сравнении с ларингоскопией.</p></sec><sec><title>Материал и методы</title><p>Материал и методы.</p><p>Первая часть исследования включала 809 первичных пациентов, которым выполняли чрескожное УЗИ гортани. Во второй части исследования у 443 пациентов до и после операции на щитовидной и околощитовидных железах выполняли УЗИ гортани и эндоскопическую ларингоскопию.</p></sec><sec><title>Результаты</title><p>Результаты.</p><p>Визуализация голосовых складок возможна у 85,9% пациентов, при этом у женщин визуализация голосовых складок возможна в 90,6%, у мужчин – в 27,9% случаев (c2 = 183,6; p &lt; 0,001). У пациентов до 40 лет визуализация голосовых складок возможна в 97,4%, у лиц старше 80 лет этот результат не превышает 57,1% (c2 = 42,4; p &lt; 0,001). Диагностическая эффективность метода ультразвуковой визуализации нарушения подвижности голосовых складок оказалась равной 91,4%. Чувствительность метода составила 62,5%, специфичность – 95,3%. Диагностическая эффективность УЗИ для диагностики пареза голосовых складок оказалась значительно более высокой – 97,7%. Рост данного показателя обусловлен прежде всего повышением чувствительности теста, составившей 88,2%. Специфичность метода также выросла и составила 98,5%. Зависимость диагностической эффективности метода ультразвуковой диагностики патологии голосовых складок от возраста пациентов оказалась статистически значимой (c2 = 6,3; p &lt; 0,05).</p></sec><sec><title>Выводы</title><p>Выводы.</p><p>Чрескожное ультразвуковое исследование голосовых складок является эффективным и удобным методом контроля функции гортани у пациентов, перенесших оперативное лечение на щитовидной и околощитовидных железах.</p></sec></abstract><trans-abstract xml:lang="en"><p>The amount of thyroid and parathyroid surgeries increases annually. Contemporary diagnostics of such a severe complication, as larynx paresis, remains nowadays actual. The main diagnostic procedure for larynx paresis is endoscopic laryngoscopy, though it has several disadvantages: this is an invasive procedure, causing discomfort to the patients, it requires local anesthesia, which may lead to allergy; some patients can’t endure this procedure because of the anatomical features. All these factors force to find out a similar and at the same time effective diagnostic procedure for revealing disturbances of vocal cords mobility.</p><p>This work represents the experience of implementation of 1252 percutaneous ultrasound examination of the vocal cords. We showed the diagnostic efficiency and convenience of this method at the endocrine surgical department.</p><sec><title>Aim</title><p>Aim.</p><p>To evaluate a possibility of percutaneous ultrasound examination for detection of vocal cords mobility after thyroid and parathyroid surgery comparing with the laryngoscopy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods.</p><p>The first stage of the work included 809 patients, who were subjected to percutaneous larynx ultrasound check-up. During the second stage 443 patients were examined before and after operative treatment by means of larynx ultrasound and endoscopic laryngoscopy.</p></sec><sec><title>Results</title><p>Results.</p><p> It is possible to visualize vocal cords in 85.9% of patients: in case of female patients it is possible to visualize vocal cords in 90.6%, and in men in 27.9% (c2 = 183.6; p &lt; 0.001). The visualization of vocal cords is feasible in 97.4% of patients under 40 years and in 57.1% in patients over 80 years (c2 = 42.4; p &lt; 0.001). Diagnostic efficiency of ultrasound visualization of vocal cords abnormality appeared to be 91.4%.The sensitivity of the method reached 62.5%, specificity 95.3%. Diagnostic efficiency of ultrasound visualization of vocal cords paresis reached 97.7%. The increase of this index is caused by sensitivity growth, which was 88.2%.
