Для этой цели апробировано нескольких молекулярных маркеров, однако ценность их применения различна.</p></sec><sec><title>Цель</title><p>Цель: количественное определение мРНК маркеров-кандидатов высокодифференцированного РЩЖ (галектина-3, Ki-67, убиквитина, HMGA2) методом полимеразной цепной реакции в режиме реального времени (РТ-ПЦР) в пунктируемом материале из узлов щитовидной железы (ЩЖ). Установить значимость данного метода исследования для диагностики злокачественных новообразований в ЩЖ на дооперационном этапе.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 55 пациентов с клиническим диагнозом узлового/многоузлового зоба. На пункционном материале проводился количественный анализ мРНК галектина-3, Ki-67, убиквитина, HMGA2 при помощи обратной транскрипции и РТ-ПЦР. Результаты ПЦР анализировали с помощью метода максимума второй производной (Second Derivative Maximum Method).</p></sec><sec><title>Результаты</title><p>Результаты. В исследовании участвовало 46 женщин (83,6%) и 8 мужчин (14,5%). Средний возраст пациентов составил 52,1 года (от 23 до 82 лет). По результатам гистологического исследования, доброкачественных образований было 35 (63,6%), злокачественных (представлены папиллярным раком) – 20 (36,4%). В структуре гистологических заключений фолликулярного рака не встретилось. После проведения ПЦР были найдены значимые различия в экспрессии мРНК гена убиквитина между злокачественными и доброкачественными узловыми образованиями ЩЖ.</p></sec><sec><title>Заключение</title><p>Заключение. Значение мРНК гена убиквитина 8,24 по результатам РТ-ПЦР обладало чувствительностью 68,4% и специфичностью 68,6% в диагностике высокодифференцированного рака на этапе проведения ТАБ-УЗИ. Исследование мРНК генов Ki-67, галектина-3, HMGA при помощи РТ-ПЦР не показало себя надежным методом для дифференциальной диагностики узлов ЩЖ на предоперационном этапе.</p></sec><sec><title>Ключевые слова</title><p>Ключевые слова: рак щитовидной железы, узловое образование, ПЦР, ПЦР в режиме реального времени, маркеры рака, галектин-3, Ki-67, убиквитин, HMGA2.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Actuality</title><p>Actuality. The differential diagnosis of follicular adenoma, follicular variant of papillary thyroid cancer and follicular thyroid cancer (TC) is one of the main topics of research. For this purpose several molecular markers were approbated, however their diagnostic effectiveness differs.</p></sec><sec><title>Aim</title><p>Aim. Quantification of candidate mRNA markers for differentiated thyroid cancer (galectin-3, Ki-67, ubiquitin, HMGA2) using polymerase chain reaction in real time (RT-PCR) of thyroid nodules pre-operative material. To evaluate the efficiency of this method for thyroid malignancy diagnostics.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 55 patients with a clinical diagnosis of nodular / multinodular goiter. A quantitative analysis of mRNA of galectin-3, Ki-67, ubiquitin, HMGA2 was performed on material puncture by reverse transcription and RT-PCR. The Second Derivative Maximum Method was used to analyze the results.</p></sec><sec><title>Results</title><p>Results. The study included 46 women (83.6%) and 8 men (14.5%). The average age of the patients was 52.1 (from 23 to 82) years old. According to the results of histological examination, there were 35 (63.6%) benign tumors, 20 (36.4%) – cancer tumors (papillary carcinoma). There were no cases of follicular cancer in the histological findings. We found significant differences in the expression of mRNA of the ubiquitin gene between malignant and benign thyroid nodules.</p></sec><sec><title>Conclusion</title><p>Conclusion. The method of ubiquitin gene 8.24 mRNA estimation has a sensitivity of 68.4% and a specificity of 68.6% in the well-differentiated cancer diagnostics.
