В клинической практике регулярно встречаются ситуации, при которых невозможно взять кровь из периферических вен для проведения лабораторных анализов. При этом проблема сравнительной оценки показателей венозной и капиллярной крови остается недостаточно изученной.</p></sec><sec><title>Цель</title><p>Цель. Провести сравнительную оценку 5 лабораторных показателей функции щитовидной железы – свободного Т3 (св.Т3), свободного Т4 (св.Т4), антител к тиреоглобулину (АТ-ТГ), антител к тиреопероксидазе (АТ-ТПО), тиреотропного гормона (ТТГ) – для выявления характера отклонений рассматриваемых параметров капиллярной крови от аналогичных показателей венозной.</p></sec><sec><title>Методы</title><p>Методы. В исследование были включены 22 клинически здоровых добровольца обоих полов, единовременно сдавших для обследования образцы капиллярной и венозной крови с соблюдением правил преаналитики. Выполнение исследований производили в течение трех часов после взятия биоматериала на анализаторе Roche Cobas e601. Для всех показателей рассчитывали средние значения с 95% доверительным интервалом (ДИ), вычисленным процедурой бутстрепа. В парах значений показателей венозной и капиллярной крови рассчитывали их разность, которую выражали в относительных (в %) единицах. Для сравнения зависимых выборок также использовали критерий Уилкоксона для разностей пар и корреляционный анализ.</p></sec><sec><title>Результаты</title><p>Результаты. Для св.Т3 (среднее отклонение 3,11%; p &lt; 0,001), св.Т4 (3,89%; p &lt; 0,001), АТ-ТГ (2,73%; p = 0,041) и АТ-ТПО (6,75%; p &lt; 0,001) продемонстрировано статистически значимое увеличение, а для ТТГ (-4,3%; p &lt; 0,001) – снижение значений для образцов капиллярной крови по сравнению с аналогичными показателями венозной. Отклонение в пределах 5% в подавляющем большинстве случаев не имеет клинического значения при постановке диагноза и выборе терапии для пациента. Все рассмотренные показатели капиллярной крови обладают сильной корреляцией (r &gt; 0,75) с аналогичными показателями венозной крови: для св.Т3 r = 0,971; св.Т4 – r = 0,993; АТ-ТГ – r = 0,958; АТ-ТПО – r = 0,836; ТТГ – r = 0,995.</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, показатели св.Т3, св.Т4 и ТТГ можно определять в капиллярной крови без какого-либо ущерба для точности клинической оценки при строгом соблюдении преаналитических правил, при этом для таких показателей как АТ-ТГ, АТ-ТПО данные отклонения могут иметь значение только при результатах, находящихся на верхней границе референсных интервалов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In clinical practice it is often impossible to draw blood from peripheral veins for laboratory testing. At the same time the problem of comparison of venous and capillary blood parameters is still insufficiently studied.</p></sec><sec><title>Aims</title><p>Aims. To compare 5 laboratory indicators of the thyroid gland function – FT3 (free T3), FT4 (free T4), TgAb (thyroglobulin antibodies), TPOAb (thyroid peroxidase antibodies), and TSH – in order to assess deviations of these parameters in venous against capillary blood.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study enrolled 22 clinically healthy patients of both sexes who had their venous and capillary blood samples simultaneously collected in compliance with pre-analytical rules. The tests were performed within three hours of sample collection on the analyzer Roche Cobas e601. Mean values were calculated for all parameters using a 95% confidence interval based on bootstrap. Differences between paired values of venous and capillary blood parameters were calculated and expressed in relative units (%). Wilcoxon test and correlation analysis were used to compare dependent samples.</p></sec><sec><title>Results</title><p>Results. A statistically significant increase in capillary against venous samples was shown for FT3 (mean deviation 3.11; p &lt; 0.001), FT4 (3.89%, p &lt; 0.001), TgAb (2.73%; p = 0.041) and TPOAb (6.75%; p &lt; 0.001), and a decrease – for TSH (-4.3%; p &lt; 0.001). A 5%-deviation is in most cases clinically insignificant for diagnosis and choosing a treatment. All the capillary blood parameters under study strongly correlate (r &gt; 0.75) with the same parameters of the venous blood: r = 0.971 for FT3; r = 0.993 for FT4; r = 0.958 for TgAb; r = 0.836 for TPOAb; r = 0.995 for TSH.</p></sec><sec><title>Conclusions</title><p>Conclusions.
